Application for Jury Membership

NAME: given name
SURNAME: family name
EMAIL: will be used to contact you
TYPE: A' Design Award features four (4) different types of jury members, please choose the type of jury member that best defines you.
TITLE: will be used in your profile, this is the title that would be displayed before your name, you could type anything including honorary titles, mostly this could also be the name of your job such as: Architect, Designer, Mr. Ms. Baron, Captain, Doctor, Lord and others. Leave blank if you do not have titles or if you do not wish to display titles.
ACADEMIC TITLE: will be used in your profile, please choose the correct title from the list of available options.
POSITION: your job position at your work, will be used in your profile, for academics this can be left blank if the academic title is already chosen above, for business people this can be for example position titles such as Architect, Engineer, CEO, Head of Design Department and similar, for journalists this could be Editor, Writer or Author or similar.
DEPARTMENT: the department you work within your institution, will be used in your profile, for academics this is the academic department, for press this is editorial department, for businesses, this can be left if there is no specific department
INSTITUTION: the name of the place where you work, will be used in your profile, for academics this is the name of university or institution, for press members this is the name of the magazine or publication, for enterprises and businesses, this is the company name.
COUNTRY: the name of the country that relates to your jury profile, please choose from the menu below.
FIRST CATEGORY: choose the first category for which you apply to become a jury member from the list below. This category should be your expertise.
SECOND CATEGORY: choose a second category for which you apply to become a jury member from the list below. This should be related to your expertise.
THIRD CATEGORY: choose a third category for which you apply to become a jury member from the list below. This should be related to your expertise.
FOURTH CATEGORY: choose a fourth category for which you apply to become a jury member from the list below. This should be related to your expertise.
FIFTH CATEGORY: choose a fifth category for which you apply to become a jury member from the list below. This should be related to your expertise. We might assign you this category if other categories are already full.
SIXTH CATEGORY: choose a sixth category for which you apply to become a jury member from the list below. This should be related to your expertise. We might assign you this category if other categories are already full.
SEVENTH CATEGORY: choose a seventh category for which you apply to become a jury member from the list below. This should be related to your expertise. We might assign you this category if other categories are already full.
EIGHTH CATEGORY: choose an eighth category for which you apply to become a jury member from the list below. This should be related to your expertise. We might assign you this category if other categories are already full.
NINETH CATEGORY: Optional. Choose a nineth category for which you apply to become a jury member from the list below. This can be any category you have a personal interest in.
TENTH CATEGORY: Optional. Choose a tenth category for which you apply to become a jury member from the list below. This can be any category you have a personal interest in.
ELEVENTH CATEGORY: Optional. Choose an eleventh category for which you apply to become a jury member from the list below. This can be any category you have a personal interest in.
TWELFTH CATEGORY: Optional. Choose a twelfth category for which you apply to become a jury member from the list below. This can be any category you have a personal interest in.

Jury Membership Application
For us to proceed with your application, we need to know you more. We usually prefer four type of jury members: 1. Academics and Scholars who could give us a research perspective. 2. Press Members and Journalists who could highlight interesting designs. 3. Business People who could identify market potential within designs. 4. Design Enthusiasts who could reflect the public opinion. During the first step of your application, please first choose one of these four (4) values and later fill all the details so that we could create a beautiful jury profile page for you, we will also need a portrait photo and we will also ask you to send a CV in PDF format for our archives. Finally, a very important aspect is for you to indicate your expertise by choosing the relevant categories for you. The first category you choose is more important because that is your expertise and your votes will have a higher weight, categories two (2) to eight (8) are also relevant and we want them to be related to your expertise. Categories from nine (9) to twelfth (12) are highly optional. The process seems long but it takes a minute, there are just many explanations to ensure you make it right. If you decide to apply via email, remember to indicate the design category numbers you choose.
